Information on the Sheila Farnan Leadership Award criteria and selection process can be found in the April 10, 2024 WIC weekly update and the WIC Local Agency webpage under Recognition and Achievements.

To nominate someone for the Sheila Farnan Leadership Award:

Complete this Sheila Farnan Award nomination form via REDCap.
Provide at least one letter of support. Up to three letters will be accepted. Letters may be from nominee's colleagues and/or public health partners. Ideally, at least one letter should be from the nominee's agency leadership.
Attach letters to completed REDCap form by July 8, 2024.
