Antibiotic Stewardship Display Materials Request Form

Thank you for your interest in display materials from the Minnesota One Health Antibiotic Stewardship Collaborative!

The materials include:

1) Question & answer content focused on antibiotic stewardship that can be inserted into a binder with sheet protectors, laminated and placed on a bulletin board, or used with existing display infrastructure

2) Infographics related to antibiotic stewardship

3) Book and film list for children and adults

- Your library could choose to gather some of these materials from your collection and have them near the display

4) Coloring page and word search puzzle pages

5) Instructions/pictures with suggestions for display layout
