Please complete this form to request the support of a navigator to help a shelter guest access COVID-19 therapeutics.

Navigators may be able to support you and/or the patient with the following activities:

  • Identify a healthcare provider to conduct medical assessment and prescribe COVID-19 therapeutics
  • Schedule medical appointments including telehealth visits
  • Arrange transportation for medical visits and prescription pick-up or delivery

People may be eligible for COVID-19 therapeutics if they meet the following criteria:

  • Patient tested positive for SARS-CoV-2 (PCR or antigen).
  • Patient has symptoms consistent with mild to moderate COVID-19 (e.g., fever, cough, sore throat, malaise, headache, muscle pain, nausea, vomiting, diarrhea, loss of taste and smell, or lower respiratory disease with SpO2 ≥94% on room air at sea level).
  • Patient is within 5-7 days of symptom onset (5 days for oral antivirals or 7 days for monoclonal antibody therapy).
  • Patient is at risk for developing severe illness from COVID-19. This includes, but is not limited to, having one or more of the medical conditions listed below. Note that the more underlying conditions a person has, the higher the risk for severe COVID-19 outcomes.
