COVID-19 Therapeutics Navigator Request Form |
Please complete this form to request the support of a navigator to help a shelter guest access COVID-19 therapeutics.
Navigators may be able to support you and/or the patient with the following activities:
- Identify a healthcare provider to conduct medical assessment and prescribe COVID-19 therapeutics
- Schedule medical appointments including telehealth visits
- Arrange transportation for medical visits and prescription pick-up or delivery
People may be eligible for COVID-19 therapeutics if they meet the following criteria:
- Patient tested positive for SARS-CoV-2 (PCR or antigen).
- Patient has symptoms consistent with mild to moderate COVID-19 (e.g., fever, cough, sore throat, malaise, headache, muscle pain, nausea, vomiting, diarrhea, loss of taste and smell, or lower respiratory disease with SpO2 ≥94% on room air at sea level).
- Patient is within 5-7 days of symptom onset (5 days for oral antivirals or 7 days for monoclonal antibody therapy).
- Patient is at risk for developing severe illness from COVID-19. This includes, but is not limited to, having one or more of the medical conditions listed below. Note that the more underlying conditions a person has, the higher the risk for severe COVID-19 outcomes.
Following is a list of conditions that may place a person at high risk for severe illness (this does not include all potential risk factors).
50 years of age or older Cancer Chronic kidney disease Chronic liver disease Chronic lung diseases Cystic fibrosis Dementia or other neurological conditions Diabetes (type 1 or type 2) |
Disabilities Heart conditions HIV infection Immunocompromised condition or weakened immune system Mental health conditions Overweight and obesity Physical inactivity |
Pregnancy Sickle cell disease or thalassemia Smoking, current or former Solid organ or blood stem cell transplant Stroke or cerebrovascular disease Substance use disorders Tuberculosis |
