The goal of the Minnesota Title V Program is to achieve optimal health and wellbeing for Minnesota's mothers, children, and families by addressing health disparities and the social conditions impacting health. It includes programs at the Minnesota Department of Health and across Minnesota's local public health agencies.

Minnesota's Title V Action Plan is an overview of how Minnesota's Title V program is addressing the needs of mothers, children, and families.
