Background
Successful and effective stroke treatment by EMS requires a commitment to continuous quality improvement. These efforts drive the quality of care delivered to stroke patients - which leads to better outcomes. EMS agencies in the state of Minnesota are an integral part of the system providing timely patient care and alerting the receiving hospitals of an incoming stroke patient.
Purpose
This award program aims to recognize and showcase successful quality improvement projects throughout the state of Minnesota's EMS agencies. We plan to acknowledge and celebrate the accomplishments of up to five applicants through an awards ceremony and on our Minnesota Department of Health (MDH) Stroke Program website.
Application Components
Quality improvement plan development
- Describe how you identified the focus area for improvement and what agency and hospital (if applicable) members were involved in the development of the plan.
- Describe the quality improvement plan, including the goal, activities, and the monitoring of performance metrics.
Quality improvement plan implementation
- Describe the steps you took to implement the quality improvement plan.
- Include any encountered challenges and how they were addressed and/or how plan was changed in response.
Quality improvement plan results
- Summarize successes and lessons learned as a result of the QI plan
- Describe results of the plan and changes (improvements) in performance metrics.
- Include a table and/or graph that showcases the change in performance metrics (if applicable).
Program specific photograph - Include at least one unique program-specific picture (i.e., EMS agency leadership team on project, EMS crew
Evaluation
We have an internal review committee that independently evaluates each application. Applications will be scored based on:
- Clarity of narratives
- Completeness of answers
- Quality of plan design and implementation
- Display of results
Acknowledgement
MDH will work with all award recipients to copy edit their stories for publication. Award recipients will receive a certificate and use of award program logo on promotional and marketing materials, including web sites. Recipients will be recognized through an awards ceremony and on the MDH Stroke Program website.
