New Arrival Notification and Screening Referral

The Minnesota Department of Health Refugee Health Program assists eligible newcomers to navigate health resources and coordinates the Refugee Health Assessment (also called the domestic medical exam). The program is requesting the new arrivals' information to help coordinate these health services. Please fill out only one form per family, as completely as you can.
